be buying the puppy that has been placed on hold for you, that is why the deposit is non-refundable).

___ Breeder guarantees that the above mentioned dog is in good health and has received the proper vaccinations and
dewormings according to the dog's age.
___ Breeder CANNOT guarantee the adult size, conformation, color or temperament.
~~~REPLACEMENTS WILL ONLY BE MADE IF ALL OF THE FOLLOWING CONDITIONS ARE MET~~~
___Seller, Lisa Smith, requires the Buyer to take your their puppy to a veterinarian, of buyer's choice, for an initial
health evaluation, within 72 hours from the date on this agreement for this “New Puppy Agreement/Health
Guarantee” to be in effect. If the puppy is not checked within the stated amount of time, the Seller/Breeder is under no
obligation to accept the return of the dog or to refund any of the purchase price of the dog. If the dog is not yet vet-
checked, the breeder is not responsible in anyway for any problems that may occur after the stated 3 day period.
Should your puppy be diagnosed with a serious health problem at time of evaluation, the Seller must be notified
immediately. Common problems such as diarrhea, wormy stools, ear mites, external parasites, allergies, coccidia,
giardia or stress due to change are NOT covered by any guarantee. A copy of the licensed veterinarian’s diagnosis
must be sent to us along with his signature, license number, phone number and address within 5 days of diagnosis.
The actions and upbringing of the dog is the responsibility of the new owner. Under no circumstances is the breeder
responsible for the dog's behavior once it leaves the breeders possession.
___ Seller guarantees for one year against hereditary or congenital life threatening or life altering birth disorders that
may interfere with the puppy’s ability to lead a normal life.
___Conditions are listed and limited to HEART DISEASE, KIDNEY/ LIVER FAILURE & BACK PROBLEMS due to birth
defects. (Veterinarian conformation is required). A licensed veterinarian must supply the breeder with a written
diagnosis and be signed wit his license number, phone number & address. (At the breeder’s request, a second opinion
may be required, from a non affiliated veterinarian, at the expense of the buyer).
___Seller requires a Necropsy (animal autopsy for verification of cause of any death within the guarantee period.
___This guarantee DOES NOT cover injuries caused by neglect, abuse or accidents.
No other conditions are covered by this contract.
__Should a birth defect develop, Seller will give another puppy to the Buyer as soon as one comes available of equal
value unless Buyer pays the difference. Buyer MUST return the puppy with all necessary paperwork at Buyer's
expense. No money will be refunded, no veterinarian, hospitalization, laboratory no medication will be paid by the
breeder, should you decide to keep the puppy. It would be your choice, to pay for all needed care and expenses.
___This guarantee does not cover any cosmetic expenses such as crooked teeth, baby teeth not falling out, navel
hernia, kink is tail, dew claws, hair loss.
__ Buyer is aware that owning a puppy, means trips to the veterinarian and understands that he/she will be responsible
for their puppy’s proper care, and to keep their pup current on all vaccinations and de-worming.
__Additional guarantee: If above stated puppy is bought as a breeder, with full AKC papers, your dog once reached
adult age does not produce, due to a birth defect, (vet conformation is required) we will give you another of equal
value. We do not refund any money nor pay for any procedures. Shipping of replacement dog will be at the original
Buyer's expense.
